4.0 APPLICABILITY
This procedure applicable to Xxx India Pvt Ltd, Xxx Plant
5.0 PURPOSE
To layout procedure to ensure that all accidents which results in injury / learning incidents are investigated.
6.0 SCOPE
To provide basic guidelines for objective information gathering and developing solutions to prevent reoccurrence of the accidents.

9.0 PROCEDURES
9.1 All accidents and learning incidents (near misses) must be investigated, no matter how minor.
9.2 Departmental head shall conduct the initial investigation as soon as possible, after an accident / incident/spill (in any such any quantity or volume that may harm man, material or environment). This report should be completed no later than the end of the shift in which the accident/incident/spill occurred. If the departmental head is not available during the time of spill/learning incident/accident/shift, the designated associate in that area will conduct the accident/incident/spill investigation and fill up the accident / Injury, learning incident or spill report. Blank reports may be obtained from the document center.
9.3 Photographs of the accident/incident /spills (in any such any quantity or volume that may harm man, material or environment) on site shall be taken, whenever possible.
9.4 The investigations shall be conducted objectively and with an open mind. Fact finding while avoiding faultfinding is crucial to the accident investigation. The parties involved in the accident/incident shall be interviewed first to gather information. Subsequently, other witnesses shall be interviewed for additional information to complete the investigation.
9.5 Head of the department shall identify and implement appropriate counter-measures immediately to eliminate the cause of the accident/incident and to prevent any recurrence. EHS Coordinator and the concerned supervisor shall jointly document the findings, observations and solutions in the Occupational Injury/ illness (OI&I) report form. The report shall then be circulated to the Management Team, Safety Committee and to other personnel as appropriate.
9.6 The EHS Coordinator shall review all lost time accident reports within 24 hours of the Accident to Regional and Intercontinental EHS contacts.
9.7 The Management Team (MT) shall review all lost time injury accident reports within 3 working days of the accident.
9.8 Safety Committee shall review all OI&I reports during the monthly meeting and ensure that hazard leading to the accident do not exist in other areas of the facility. Ensure that the root cause of the accident / learning incident or spill has been determined.
9.8.1 The EHS Coordinator shall follow up to ensure that the corrective actions are implemented as documented in the OI&I report.
9.9 The EHS Coordinator shall also file appropriate reports through the BaxHealth system and Non-conformance log.
9.10 The Accident/ Injury and the Learning Incident Report should be filled in triplicate. One copy each will be distributed as below:
-Own Department
-Medical & EHS Department
-HR Department
10.0 FOLLOW-UP ON ACCIDENT VICTIM
10.1 The EHS Coordinator is required to contact the injured/affected employee(s) at least weekly by telephone or visit.
10.2 The company doctor will review each case to ensure all injured/affected employee(s) are fit for work before returning to active duty. However, if the injured/affected employee(s) are granted medical leave to rest before returning to work, it is not necessary for the injured/affected employee(s) to be reviewed by the doctor again.
